Hi, for the two of the schools I am applying to they have a Recommendation form for recommenders to fill out and send to the school. It is in a question and answer format and includes a rating chart. Are my recommenders supposed to fill this out, and then also send in a letter? It isn't clear in the application. Thanks!

A completed form should suffice if a form is included in the application package. HOWEVER, it is always a good idea to contact the admissions office directly to ask for clarification. You would not want to get dinged over something as simple as an LOR.
